Requests for Eligibility List Restoration


Per the PRC’s Administrative Rules, candidates may be removed from a classified eligibility list for one of the following reasons:
  • At the request of the eligible candidate;
  • After three certifications or considerations without receiving an appointment;
  • Failure to pass a pre-employment background check and/or drug or alcohol screen;
  • Failure to appear for an interview;
  • Inability to locate the eligible at his or her last known address;
  • Practice or attempt to practice any deception in his or her application or in securing eligibility or appointment; and
  • Any just or reasonable cause that is job-related and non-discriminatory.

If you received notice that your name was being removed from an eligibility list and you believe this was in error, you may make a request for eligibility list restoration by completing and submitting the form via the link at the bottom of the page, following these procedures:
  • You must explain why you think you should not have been removed from the eligibility list.
  • You should include any evidence that supports your claim.

You must complete and submit the form within five (5) calendar days after receiving your original notification of removal. Only requests using this form and submitted by the deadline will be considered.

The PRC will review your submission and make a final decision. The result of that decision will be sent to you via email. Please note that restoration to the eligibility list is within the sole discretion of the PRC.
