Requests for Eligibility List Restoration

Per the PRC’s Administrative Rules, candidates may be removed from a classified eligibility list for one of the following reasons:

  • At the request of the eligible candidate;
  • After declining a conditional offer for the position;
  • After three certifications or considerations without receiving a conditional offer;
  • Failure to pass a pre-employment background check and/or drug or alcohol screen;
  • Failure to appear for an interview;
  • Inability to contact the candidate via the contact information on file with the Commission;
  • Practice or attempt to practice any deception in their application or in securing eligibility or appointment; or
  • Any just or reasonable cause that is job-related and non-discriminatory.

If you received notice that your name was being removed from an eligibility list and you believe this was in error, you may make a request for eligibility list restoration. Please read all the information below, then complete and submit the Request for Eligibility List Restoration form via one of the methods listed at the bottom of this page.

Deadline and Instructions

  • You must complete and submit the form within five (5) calendar days after receiving your original notification of eligibility list removal. Only requests using the proper form and submitted by the deadline will be considered.
  • On your form, you must explain why you think you should not have been removed from the eligibility list. If you have supporting documentation or other evidence to support your claim, you should submit it along with the form.
  • The PRC will review your submission and make a final decision. The result of that decision will be sent to you via email. Please note that restoration to the eligibility list is within the sole discretion of the PRC.

You can also print the form and write in the information. If you fill out the form on your computer, make sure you save it before submitting it. Once you complete the form, you can submit it along with any supporting documentation by one of the following methods:

  • Via email to PRCEmploymentTesting@cuyahogacounty.us. This is the preferred method to submit your form and documentation.
  • Via fax at (216) 443-3694.
  • In person or via U.S. mail to 9830 Lorain Avenue, Cleveland, Ohio 44102.
    Please note: if you mail your form and documentation, the PRC must receive it by the deadline. Submissions that are postmarked on or before the deadline but received after it will not be considered.

If you can’t download or complete the form, you can submit your request through the web form at the bottom of the page. However, this web form does not support uploading any files, so you will not be able to attach any supporting documentation. If you choose to use the web form and have any supporting documentation you’d like to include, you must submit it using one of the above methods (via email, fax, in person, or U.S. mail). Supporting documentation submitted after the deadline will not be considered when reviewing your request.
